Key Responsibilities:

The role of the Patient Care Support Coordinator involves managing phone inquiries from patients, families, healthcare professionals, and physicians regarding patient care needs. This position requires professional and efficient handling of visitors, telephone communications, clerical tasks, and customer service across all business lines. The coordinator will oversee Home Health Certification, Recertification, census, and care plan management activities. Responsibilities also include the proper routing of faxes to ensure timely processing according to company protocols.

Essential Duties:

  • Respond to inquiries from patients and caregivers with professionalism; document all interactions in the patient chart.
  • Triaging, interpreting, documenting, and adhering to established protocols for all business lines.
  • Assess patient needs and communicate resolutions through the system.
  • Relay medical visit requirements to the Scheduling Department.
  • Review patient charts post-visits and process physician orders through the appropriate home health agency or vendor.
  • Receive critical lab value reports and communicate with the medical team for follow-up actions.
  • Document physician orders in the patient chart and execute actions as directed.
  • Record any patient or caregiver complaints and take immediate corrective actions when possible.
  • Provide written reports to supervisors.
  • Maintain patient confidentiality at all times.
  • Manage and archive medical records based on clinical urgency, following standardized naming conventions.
  • Collaborate with all members of the healthcare team and office staff in the designated service area.
  • Perform routine office tasks necessary for maintaining a professional office environment.

Qualifications:

Required Skills and Experience:

  • High School Diploma or GED.
  • Understanding of compliance with local, state, and federal laws regarding licensure and certification in Home Health Care.
  • Familiarity with home health care referral criteria and services.
  • Stay informed about changes in the home health care field and share relevant information with team members.
  • Ensure coordination and delivery of home health services within the practice.
  • Proficient in Microsoft Excel, Word, and Outlook.
  • Strong organizational, communication, and time management skills.
  • Ability to multitask, self-motivate, and maintain a professional demeanor.
  • Adaptability to new systems and application of guidelines/procedures.
  • Problem-solving skills with the ability to assess inquiries and prioritize resolutions.
  • Teamwork skills and the ability to maintain relationships with patients, caregivers, and colleagues.

Preferred Skills and Experience:

  • Medical/technical certification or practical nursing license.
  • Experience with Electronic Medical Records (EMR).
  • Two or more years of customer service experience in a healthcare setting.
  • Knowledge of HarmonyCares practice protocols for patient chart maintenance and customer service.
