Job Summary

Key Responsibilities
  • Provide individual and group therapy sessions to patients, utilizing evidence-based interventions and treatments.
  • Conduct suicide assessments and make recommendations to medical providers and patients, determining if Crisis Response Services are appropriate for patient safety.
  • Work in collaboration with the patient's primary care team to develop and implement care plans that address the patient's physical and mental health needs.
  • Attend team huddles and communicate regularly with the care team to ensure seamless coordination of care.
  • Provide education and support to patients, families, and staff on mental health, wellness, and related issues.
Requirements
  • Master's degree in a related field, such as social work or counseling.
  • LCSW, LCPC, or candidate licensure in good standing.
  • Previous experience working in a team setting or integrated behavioral health model is preferred.
