Pediatric Home Team Nurse

We are seeking a highly skilled and compassionate Pediatric Home Team Nurse to join our team at Angels of Care Pediatric Home Health. As a key member of our Home Team, you will play a vital role in providing high-quality patient care to infants, children, and adolescents in their homes.
Key Responsibilities- Assess patients' post-acute needs and develop individualized care plans in collaboration with the multi-disciplinary Home Team and discharge care providers.
- Provide skilled nursing care, including administration of medications, treatments, and therapies, to patients in their homes.
- Conduct ongoing patient assessments and coordinate care with other healthcare professionals to ensure continuity of care.
- Educate family members and caregivers on patient clinical care to enhance positive outcomes.
- Participate in preventative initiatives to protect quality of care for patients.
- Provide interim care to patients experiencing gaps due to call-offs, vacations, and staff shortages.
- Collaborate with support staff to serve as a liaison and patient advocate on complicated cases.
- Active Texas RN or LPN license.
- Active CPR certification.
- Trach/vent experience preferred.
- Home care experience preferred.
- Strong oral and written communication skills.
- Ability to work as a team member and adapt to changing situations.
