Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Care team as a Primary Care Nurse. As a key member of our team, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with our Population Health Case Managers and other healthcare professionals to achieve optimal patient outcomes.

Key Responsibilities
  • Provide primary nursing care to patients in their homes, following physician orders and adhering to our organization's policies and procedures.
  • Develop and implement individualized care plans, working closely with patients, families, and healthcare teams to ensure seamless care coordination.
  • Supervise and support Home Healthcare Aides and LPNs as needed, promoting a culture of teamwork and collaboration.
  • Utilize motivational interviewing and health coaching techniques to empower patients to achieve their health goals.
  • Manage patient schedules and visit utilization to optimize agency performance and achieve operational efficiencies.
  • Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure comprehensive care and coordination.
Requirements
  • Graduate of an approved Nursing education program, BSN preferred.
  • Current Registered Nurse licensure in the state of practice.
  • Minimum of one year professional nursing experience, with Med-Surg, ER, Urgent Care, ICU, Critical Care, Progressive Care, or Step-Down Unit experience.
  • Strong computer and technology skills required.
  • Current Driver's license and reliable transportation.
