Job Summary:

As a Home Health Nurse with Kaleida Health, you will play a vital role in coordinating the total Plan of Care for patients in the Home Care setting. This involves assessing, planning, implementing, and evaluating patient care to ensure the highest level of quality and safety.

Key Responsibilities:

Coordinate patient care with the healthcare team to ensure seamless delivery of services

Assess patient needs and develop individualized care plans to meet those needs

Implement and evaluate patient care plans to ensure quality and safety

Requirements:

Current license as a Registered Nurse (RN) in the state of New York

1-5 years of experience in home health care or a related field

Strong leadership and communication skills
