As a skilled Physical Therapist with CenterWell Home Health, you will play a vital role in helping patients recover from injuries, illnesses, or surgeries, and regain their independence. Our team of dedicated professionals is committed to delivering personalized, comprehensive care that addresses the physical, emotional, and social needs of our patients.

Key Responsibilities:
  • Develop and implement individualized treatment plans to help patients achieve their maximum potential.
  • Conduct thorough assessments to identify patients' strengths and weaknesses, and create tailored plans to address their unique needs.
  • Collaborate with our multidisciplinary team to ensure seamless care coordination and effective communication.
  • Provide education and training to patients and their families on proper techniques and strategies for managing their conditions.
  • Monitor patients' progress, make adjustments to treatment plans as needed, and ensure timely documentation of all care activities.
Requirements:
  • Degree from an accredited Physical Therapy Program (approved by the APTA)
  • Minimum of one year physical therapy experience preferred
  • Current and unrestricted Physical Therapy license
  • Current CPR certification
  • Strong organizational and communication skills
  • A valid driver's license, auto insurance, and reliable transportation are required.
