Senior Social Worker

We are seeking a highly skilled Senior Social Worker to join our team at the University of Washington Medical Center - Northwest. As a Senior Social Worker, you will play a critical role in providing exceptional patient care and support to our patients and their families.
Job Summary:The Senior Social Worker will work collaboratively with the healthcare team to address the social and emotional needs of patients, provide support and advocacy, and facilitate the progression of care. This role requires a strong understanding of social work principles, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment.
Responsibilities:- Provide psychosocial assessments, treatment planning, and therapeutic interventions to patients and their families.
- Collaborate with the healthcare team to coordinate care and ensure seamless transitions between departments.
- Develop and implement discharge plans to ensure patients receive necessary support and resources.
- Provide crisis intervention and resource referral services to patients and their families.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Masters of Social Work degree from a program accredited by the Council on Social Work Education.
- One year of full-time social work experience in a healthcare setting or equivalent.
- Must hold one of the following credentials issued by the state of Washington: Agency Affiliated Counselor, Licensed Social Work Associate - Advanced, Licensed Social Work Associate - Independent Clinical, Licensed Advanced Social Worker, or Licensed Independent Clinical Social Worker.
