Senior Social Worker

We are seeking a highly skilled and compassionate Senior Social Worker to join our team at the University of Washington Medical Center - Northwest. As a Senior Social Worker, you will play a critical role in providing exceptional patient care and support to our patients and their families.
Job Summary:The Senior Social Worker will work collaboratively with the healthcare team to assess, plan, and implement social work interventions to address the complex needs of our patients. This includes providing emotional support, connecting patients with community resources, and advocating for their rights and interests.
Responsibilities:- Conduct comprehensive psychosocial assessments of patients and their families to identify their strengths, needs, and goals.
- Develop and implement individualized treatment plans to address the social and emotional needs of patients.
- Provide emotional support and counseling to patients and their families to help them cope with their medical conditions and treatment plans.
- Connect patients with community resources and services to support their recovery and well-being.
- Advocate for patients' rights and interests, ensuring that their needs are met and their voices are heard.
- Collaborate with the healthcare team to coordinate care and ensure seamless transitions between departments.
To be successful in this role, you will need:
- A Master of Social Work degree from a program accredited by the Council on Social Work Education.
- At least one year of full-time social work experience in a healthcare setting.
- Strong assessment and treatment planning skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with the healthcare team.
