- Assess and monitor the physical, mental, and emotional states of patients, providing individualized care to meet their unique needs.
- Administer medications to patients, ensuring accurate dosing and adherence to treatment protocols.
- Assist with exercise, range of motion, and nutritional activities to promote patient mobility and independence.
- Provide personal care to patients, including bathing and toileting, while maintaining a clean and safe environment.
- Use medical supplies and equipment to prevent wound infections and promote healing.
- Document patient progress and communicate effectively with healthcare providers, families, and social workers.
- Current LPN licensure in the state of employment.
- Basic Life Support (BLS) certification.
- Experience working in long-term care or skilled nursing settings.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
