We are seeking a skilled and compassionate LPN Visit Nurse to join our team at Almost Family - La Crosse PD. As a LPN Visit Nurse, you will have the opportunity to provide high-quality care to patients in their homes, making a meaningful difference in their lives.
Job SummaryThe LPN Visit Nurse is responsible for providing direct nursing care to patients in their homes, as prescribed by the physician. This includes recording clinical and progress notes, coordinating patient care activities, and providing education to patients and their families.
Responsibilities- Provides high-quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing.
- Documents patient visits thoroughly and completely per regulatory and payer requirements.
- Actively participates in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
- Ensures confidentiality of patient health information per agency policy.
- Current LPN licensure in the state of practice.
- Current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
