We are seeking a dedicated and compassionate Part-Time LPN to join our team at Home Matters Caregiving Gold Coast. As a licensed practical nurse, you will play a vital role in delivering exceptional care and assistance to our clients and their families.

Job Summary:

We are looking for a skilled and compassionate LPN to conduct comprehensive assessments of clients' health status and develop individualized care plans. You will work collaboratively with our team of professional caregivers to ensure the delivery of high-quality care and provide emotional support and counseling to clients and their families.

Responsibilities:

  • Conduct comprehensive assessments of clients' health status and develop individualized care plans.
  • Coordinate with the interdisciplinary team to implement and modify care plans as needed.
  • Educate clients and their families about health conditions, treatment plans, and self-care techniques.
  • Provide emotional support and counseling to clients and their families to promote well-being and coping strategies.
  • Maintain accurate and up-to-date client records, including assessments, care plans, progress notes, and medication administration records.
  • Prepare and submit reports as required by regulatory agencies and company policies.
  • Collaborate with other healthcare professionals, including physicians, therapists, and caregivers, to coordinate care and ensure continuity of services.
  • Communicate effectively with clients, family members, and team members to promote collaboration and achieve positive outcomes.

Qualifications:

  • Valid LPN license in the state of Connecticut
  • Minimum of 5 years of nursing experience, preferably in home care or a related setting.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities - verbal and written.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to providing compassionate care and promoting client well-being.
