Key Responsibilities- Manage new referrals for the Health Home program, ensuring timely and accurate processing.
- Conduct intakes and complete intake documents and initial assessments, ensuring compliance with program requirements.
- Verify Medicaid and program enrollment eligibility through the use of EPACES, MAPP, and PSYCKES systems.
- Admit new intakes in the Foothold Care Management electronic health record and upload supporting documents as per Lead Health Homes policies and procedures.
- Support Care Management and team activities as needed, ensuring seamless coordination of services.
- Conduct community-based work to promote program services and enroll new clients.
- Participate in department meetings and clinical reviews, ensuring ongoing education and professional development.
- Perform administrative tasks as needed, maintaining accurate and up-to-date records.
- At least 1 year of related experience in a healthcare or social services setting.
- High school diploma or equivalency required, with a focus on customer service and communication skills.
- Strong socio-cultural identification with the target population, with a commitment to delivering culturally sensitive services.
- Excellent oral and written communication skills, with the ability to work effectively with diverse stakeholders.
- Detail-oriented and organized, with a strong focus on accuracy and attention to detail.
