About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Hospice Home Care team at EvergreenHealth. As a Hospice Home Care Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.

Responsibilities
  • Provide direct patient care, assessing the comprehensive nursing needs of assigned patients and developing individualized care plans.
  • Coordinate patient and family education, ensuring they are empowered to make informed decisions about their care.
  • Initiate patient start of care through established admission processes, working closely with physicians and other team members to develop and implement patient plans of care.
  • Continuously evaluate patient care, making adjustments as needed to ensure optimal outcomes.
  • Communicate effectively with patients, families, and healthcare team members, ensuring seamless care coordination.
Requirements
  • Bachelor's degree in Nursing (BSN) required within 5 years of employment.
  • Licensed as a Registered Nurse in the State of Washington.
  • Current Healthcare Provider BLS certification.
  • Current Washington State Driver's license and proof of insurability.
