RN-Hospice Home Care Specialist

As a Hospice Home Care RN, you will provide comprehensive nursing care to patients in their homes, focusing on symptom management, pain control, and improving quality of life. You will work collaboratively with an interdisciplinary team to develop and implement individualized care plans, ensuring patient-centered care and family support.
Key Responsibilities:- Assess and evaluate patient needs, developing and implementing individualized care plans
- Provide direct patient care, including medication management, wound care, and vital sign monitoring
- Collaborate with physicians, social workers, and other healthcare professionals to ensure comprehensive care
- Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to care plans
- Participate in ongoing education and training to stay current with best practices and evidence-based care
- Current RN licensure in the State of Washington
- BSN degree or commitment to begin a BSN program within three years
- Current Healthcare Provider BLS certification
- Valid Washington State driver's license and proof of insurability
