ROLE OVERVIEW

Deliver compassionate palliative nursing care and support to pediatric patients and their families in a home setting.

KEY RESPONSIBILITIES:
  • Evaluate the physical, emotional, and spiritual needs of patients and their families.
  • Administer skilled nursing services as directed by the patient's physician.
  • Monitor pain and symptoms, providing guidance and support to manage these challenges.
  • Act as a liaison with the patient's healthcare providers, sharing relevant information regarding patient needs.
  • Offer emotional support to patients and families throughout their care journey.
  • Refer patients and families to additional services as necessary and ensure follow-up.
  • Educate patients and families on home care practices.
  • Maintain accurate documentation and reports related to patient care.
  • Engage in care conferences and bereavement support meetings.
  • Participate in community outreach and public relations activities as needed.
  • Contribute to quality assurance initiatives and team meetings.
  • Oversee the activities of Home Health Aides in relation to patient care.
QUALIFICATIONS:
  • Active RN license in the State of Ohio.
  • Bachelor's degree or equivalent experience.
  • 3-5 years of experience in pediatric nursing or hospice care, with a willingness to transition into pediatrics.
  • Preferred experience in pain management, oncology, public health, or mental health.
  • Strong knowledge of pediatric growth and development, assessment skills, and pharmacology.
  • Availability to work a minimum of 40 hours per week, including potential 10-hour shifts.
  • Evening and weekend on-call responsibilities as required.
  • Valid driver's license and reliable transportation with appropriate insurance coverage.
  • Service area may include multiple counties, with efforts made to assign nurses close to their residence.
