Job Summary

We are seeking a highly skilled and compassionate Dialysis Resource Navigator to join our team at Fresenius Medical Care North America. As a key member of our healthcare team, you will play a vital role in exploring, recommending, and coordinating community resources and social services programs for our kidney dialysis patients.

Key Responsibilities
  • Drive the company culture through values and customer service standards.
  • Provide outstanding customer service to all external and internal customers.
  • Develop and maintain effective relationships through effective and timely communication.
  • Take initiative and action to respond, resolve, and follow up regarding customer service issues with all customers in a timely manner.
  • Collaborate with local offices to obtain Medicare and Medicaid Insurance Assistance (non-commercial insurance assistance).
  • Coordinate Pharmacy Program benefits and prescription assistance programs.
  • Social service and community resource navigation and referral, including emergency food, housing, and shelter resources.
  • Assist with transportation referrals, authorizations, and re-certifications.
  • Identify appropriate community social service agencies and other resources to assist patients and families, as directed by the MSW.
Requirements
  • Bachelor's Degree in Social Work (BSW) required.
  • 0-2 years' of Healthcare experience or related with specific knowledge of and experience with navigating community resources.
  • Strong oral and written communication skills.
  • Detail-oriented.
  • Strong organizational and time management skills.
  • Proficient with PCs and Microsoft Office applications.
