Job Summary:

Old Pueblo Community Services is seeking a skilled Case Manager to join our team. As a Case Manager, you will work closely with clients to develop and implement personalized case plans, providing support and guidance to help them achieve their goals.

Key Responsibilities:

  • Conduct intake assessments and develop case plans in collaboration with clients.
  • Maintain accurate and confidential resident files, adhering to HUD and HIPAA standards.
  • Interview clients to identify their needs and develop strategies to address them.
  • Refer clients to relevant services and organizations as needed.
  • Monitor client progress and provide regular feedback and support.
  • Attend staff meetings and collaborate with supervisors as required.

Requirements:

We are seeking a highly motivated and organized individual with a Master's or Bachelor's degree in a related field, or equivalent experience. The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment.
