Key Responsibilities:
  • Evaluate and assess new patients to develop effective care plans.
  • Create and update comprehensive short and long-term patient care plans to ensure optimal outcomes.
  • Educate families and patients about resources and healthcare options to empower informed decision-making.
  • Collaborate with other RNs, doctors, and social workers to provide seamless care coordination.
  • Properly document and submit medical records to ensure accurate patient information.
  • Communicate with insurance companies to collaborate or appeal decisions to secure necessary resources.
  • Analyze results, monitor variances, identify trends, and recommend actions to management to improve financial status.


Requirements:

* BLS certification

* 2 years of recent case management experience

Physical Demands:

* Ability to stand and walk for long periods of time
* Ability to carry up to 10 pounds of weight
* Ability to bend, lean, and stoop without difficulty

Essential Skills:

* Excellent oral and written communication skills to effectively communicate with patients, families, and co-workers
* Ability to manage a fluctuating caseload during times of high census and work efficiently in a collaborative environment
