About the Role

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Provide professional nursing services under the direction of a RN, adhering to state and national standards of care.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Document patient progress, maintain accurate records, and communicate effectively with patients, families, and healthcare providers.
Requirements
  • Graduate of an accredited LPN program.
  • Current LPN licensure in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN in a clinical setting, preferably in home health or hospice care.
