Pediatric Home Health RN or LPN Job Description

UnityPoint Health is seeking a dedicated and compassionate Pediatric Home Health RN or LPN to join our team. As a Pediatric Home Health Nurse, you will work one-on-one with children and their families to develop personalized care plans that meet their unique needs.

Responsibilities:
  • Provide skilled nursing care to pediatric-young adult patients with special health care needs in their homes.
  • Perform prescribed activities directed under the patient's physician's care, including patient assessments, feedings, respiratory system management, medication administration, activities of daily living, and therapy programs.
  • Assist in care coordination with the care team and may be responsible for transportation of patients.
Qualifications:
  • Graduate of a State Board-approved program for Licensed Practical Nurses or Registered Nurses and valid license as Licensed Practical Nurse or Registered Nurse.
  • Strong computer skills.
  • Ability to understand and apply guidelines, policies, and procedures.
  • Maintenance of an acceptable driving record.
  • Ability to work as a collaborative team member.
