Responsibilities- Provide comprehensive medical care, including evaluation, assessment, and management of terminal illnesses, in compliance with state Nurse Practice Act and under the supervision of a collaborating physician.
- Conduct thorough history and physical examinations to assess patients' medical conditions and health histories.
- Complete hospice face-to-face visit requirements to assess eligibility for certification and recertification for hospice care.
- Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
- Maintain accurate and detailed clinical notes, ensuring compliance with all state and federal regulations.
- Communicate effectively with physicians and participate in interdisciplinary group meetings to address patients' medical and emotional needs.
- Ensure compliance with all state and federal legal and regulatory requirements.
- Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.
- Current DEA license, if applicable to state regulations.
- Current nursing license in state of practice.
- Current CPR certification.
- Master's degree with a minimum of one year NP experience.
- One year hospice or palliative care experience preferred.
- Accreditation from an approved certifying body for advanced practice nursing as required by state of practice.
- Certification in a specialist area preferred (e.g., Hospice and Palliative Nursing (CHPN) or Geriatrics).
- Commitment to clinical and documentation excellence.
