About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team at Haven Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to patients and their families in the comfort of their own homes.

Responsibilities
  • Conduct comprehensive assessments of patients and families to determine their unique needs and develop personalized care plans.
  • Provide expert nursing care, including administering medications, treatments, and performing various medical procedures as needed.
  • Collaborate with the interdisciplinary team to coordinate patient care, including developing and implementing care plans, and communicating with healthcare providers and family members.
  • Provide education and support to patients and families on various aspects of care, including pain management, symptom control, and end-of-life care.
  • Supervise and delegate tasks to hospice aides as needed, ensuring that patients receive the highest level of care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
Requirements
  • Graduate of an accredited school of nursing with a current Registered Nurse licensure in the state.
  • One year of hospice experience preferred, but not required.
  • Excellent communication, problem-solving, and leadership skills.
  • Ability to work independently and as part of a team.
