AFC Case Manager

At Cape Abilities, we are seeking a compassionate and dedicated AFC Case Manager to join our team. As a key member of our nursing and care management team, you will provide social work services that facilitate the screening of new family homes and participants, and assist in matching and placement services to participants.
Key Responsibilities:- Provide case management services within established departmental policies and procedures.
- Assess and provide psychosocial evaluations of potential participants and caregivers.
- Conduct monthly/bi-monthly site visits.
- Train caregivers, conduct annual home inspections, and develop and update safety plans as needed.
- Prepare and submit individual service plan (ISP) goals and assess individuals funded by the Department of Developmental Services (DDS).
- Assist program participants in securing and maintaining Mass Health coverage, social security entitlements, food stamps, and Section 8 housing as needed.
- Enter appropriate and timely documentation of case activity in participant's records.
- Develop family care placement plans based on social work practice with community health nurse; facilitate matching process with potential participants and host families.
- Provide casework/case management services to ongoing caseload including routine follow-up services, discharge planning, and crisis intervention. Provide individual, group, and family counseling as indicated and required by participant and host family need.
- Maintain professional growth and development through approved seminars, workshops, and professional affiliations to keep abreast of the latest trends in the field.
- Provide on-call coverage on weekends as scheduled.
- Collaborate with other nursing staff as needed.
- Serve as mandated reporter for suspected cases of abuse and/or neglect to the Disabled Persons Protection Commission (DPPC).
- Bachelor's degree.
- Social worker license from the Massachusetts Board of Registration in Social Work.
- Minimum 2 years prior experience working with elderly or disabled adults.
- Bachelor's degree plus 2 years clinical experience in the care of elderly or disabled individuals may be substituted for the above requirements.
- Attention to detail.
- Valid driver's license in the state of Massachusetts required.
- Successful completion of all state and Agency mandated trainings required upon hire.
- Frequent sitting and walking.
- Ability to travel in a car for prolonged periods.
