Job Overview

Mozaic is seeking a skilled Service Coordinator to join our team in Cayuga County. This role provides supervision and oversight to Direct Support Professionals (DSPs) in the Site Supervisor's absence, ensuring the health and safety of individuals supported and maintaining Life Plan documentation.

The ideal candidate will have a high school diploma or equivalent, a minimum of three years of experience working with individuals with developmental disabilities, and one year of supervisory experience. Certification in Medication Administration is required.

Key Responsibilities

  • Deliver person-directed services while ensuring the health and safety of individuals supported
  • Maintain accurate financial documentation and follow the program budget
  • Ensure services are provided according to individuals' Life Plans with all necessary documentation completed contemporaneously
  • Oversee staff training and ensure assigned personnel are fully equipped for their DSP duties
  • Maintain effective relationships with service providers, community resources, and families
  • Ensure the site remains a safe and clean environment, addressing any areas of concern promptly
