Job Overview

Cayuga Counseling Services Inc is seeking a highly skilled Care Manager - Mental Health Advocate in Cayuga County to serve as the primary point of contact for enrolled children and their families.

Key Responsibilities
  • Develop and maintain relationships with families, foster care providers, and community partners to ensure comprehensive, integrated physical and behavioral health care delivery.
  • Conduct thorough assessments to evaluate the needs and strengths of children and families, developing individualized care plans that address their unique requirements.
  • Coordinate and facilitate regular family team meetings, ensuring seamless communication among caregivers, healthcare providers, and other relevant stakeholders.
  • Promote system-wide collaboration, advocating for necessary services and linkages to support the well-being of enrolled children and families.
Requirements

To succeed in this role, you must possess:

  • A Bachelor's degree in psychology, sociology, human services, or a related field, accompanied by at least two years of experience in mental health or a related field, or;
  • A Master's degree in a relevant field, along with one year of experience, or;
  • A Licensed Registered Nurse with two years of experience in mental health or a related field.

You should also be proficient in using various software applications, including Windows, Outlook, Adobe Acrobat, Word, and Excel. Additionally, having a valid New York State Driver's License is mandatory.
