Salary RangeThe estimated salary for this position ranges from $60,000 to $80,000 annually, depending on experience and qualifications. This compensation package reflects the importance we place on attracting and retaining top talent in the industry.
Job Description: Tailored Care ManagersWe are seeking highly motivated and compassionate individuals to join our team as Tailored Care Managers. In this role, you will be responsible for coordinating comprehensive care services for individuals with IDD, MH, and SA needs. Your primary objective will be to promote whole-person care, foster high-functioning integrated care teams, and drive towards better health outcomes.
You will work closely with healthcare providers, families, and caregivers to develop personalized care plans that address the unique needs and goals of each individual. Your expertise will help bridge gaps in care, facilitate communication, and ensure seamless transitions between service providers.
- Care Coordination: Develop and implement effective care coordination strategies to ensure timely access to necessary services and supports.
- Comprehensive Assessments: Conduct thorough assessments to identify individuals' strengths, needs, and preferences, informing care plan development and goal setting.
- Care Plan Development: Create and update care plans that reflect individuals' goals, priorities, and preferences, ensuring alignment with their values and lifestyle.
- Collaboration and Communication: Foster strong relationships with healthcare providers, families, and caregivers through regular communication, updates, and education.
- Bachelor's degree in a human services field or related discipline.
- Minimum 2 years of experience in care management, social work, or a related field.
- Excellent communication, interpersonal, and problem-solving skills.
- Familiarity with Medicaid and NC State DHHS Provider Manual requirements.
- Ability to work independently and as part of a multidisciplinary team.
- Valid driver's license and reliable transportation.
