Patient Care Specialist

Kaiser Permanente seeks a skilled Patient Care Specialist to provide physical therapy services in Los Angeles, California.
About the Role- This position involves evaluating patients and developing treatment plans to restore their abilities to move and perform daily activities.
- You will work closely with other healthcare professionals to ensure comprehensive patient care.
- Evaluate patients' conditions to determine the best course of treatment.
- Develop and implement individualized treatment plans to achieve set goals.
- Administer or prescribe modalities and exercise programs as needed.
- Clinically reason to assess patient progress and adjust treatment plans accordingly.
- Regularly reassess clinical signs and symptoms to determine treatment effectiveness and patient progress.
- Aptitude for working in a Labor/Management Partnership environment.
- Completion of an accredited PT training program with a Bachelors, Masters, or Doctorate degree in Physical Therapy.
- Physical Therapist License - Applicant (California) required at hire OR Physical Therapist License (California) required at hire.
- National Provider Identifier required at hire.
- Basic Life Support required at hire from American Heart Association.
