Key Responsibilities:

  • Conduct thorough psychosocial assessments with patients and families to identify needs and develop comprehensive plans.
  • Document assessments, interventions, and transition plans clearly and accurately in electronic medical records.
  • Collaborate with healthcare teams to coordinate patient care, including emotional support, financial issues, and care transitions.
  • Facilitate decision-making processes for patients without surrogates or decision-making capacity.
  • Provide support and intervention for mental health issues, substance abuse, and end-of-life care discussions.
  • Assess and address social determinants of health, including housing instability.
  • Participate in quality improvement initiatives and training activities to enhance skills and knowledge.
  • Develop and lead group sessions or educational programs to support patient and family needs.
  • Maintain licensure and credentials for revenue-generating services.
  • Oversee psychiatric transfers and other assigned duties.

Requirements:

Minimum Qualifications

  • Master's degree in social or behavioral science or related health specialty.
  • Current State of IL Licensure as a Licensed Social Worker or Licensed Clinical Social Worker, or three years of relevant experience.
