UI Health is seeking a highly skilled Social Worker to join our team as a Psychosocial Assessment Specialist. In this role, you will be responsible for conducting comprehensive psychosocial assessments with patients and families, documenting assessments and interventions, and coordinating various aspects of patient care.
Key Responsibilities:
- Conduct thorough psychosocial assessments with patients and families to identify needs and develop comprehensive plans.
- Document assessments, interventions, and transition plans clearly and accurately in electronic medical records.
- Collaborate with healthcare teams to coordinate patient care, including emotional support, financial issues, and care transitions.
- Facilitate decision-making processes for patients without surrogates or decision-making capacity.
- Provide support and intervention for mental health issues, substance abuse, and end-of-life care discussions.
- Assess and address social determinants of health, including housing instability.
- Participate in quality improvement initiatives and training activities to enhance skills and knowledge.
- Develop and lead group sessions or educational programs to support patient and family needs.
- Maintain licensure and credentials for revenue-generating services.
- Oversee psychiatric transfers and other assigned duties.
Requirements:
Minimum Qualifications
- Master's degree in social or behavioral science or related health specialty.
- Current State of IL Licensure as a Licensed Social Worker or Licensed Clinical Social Worker, or three years of relevant experience.
