We are seeking a skilled Patient Care Specialist to join our team at Wellpath. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our patients.
Key Responsibilities- Provide direct patient care, including basic nursing services, medication administration, and communication skills for patient care and education.
- Collect and contribute to the evaluation of individualized interventions for mental health patients in accordance with their care or treatment plan.
- Maintain communication with healthcare professionals regarding patients' mental health needs while maintaining confidentiality.
- Transcribe doctor's orders, assist with program admission and discharge paperwork, and manage and organize mental health clinical documentation and records.
- High school diploma and specialized training for licensing/certification(s)
- 1-3 years post-licensure experience required
- Basic knowledge of all psychotropic medications being dispensed
- Correctional mental health experience is preferred
- Current (and must maintain) unrestricted LPT license, certification or registration in state of practice
- Current (and must maintain) lifesaving BLS certification through AHA
