Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at LHC Group. As a LPN, you will play a vital role in providing high-quality patient care and performing technical skilled care in the patient's home.

Responsibilities:
  • Provide high-quality clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the healthcare team.
  • Continually observe and report the patient's nursing needs to supervising RN.
  • Communicate timely and effectively with physicians, patients, family members, and appropriate agency staff regarding any patient care issues or needs.
  • Document patient visits thoroughly and completely per policy and payer requirements.
  • Actively participate in the performance improvement plan and work to reduce patient hospitalizations and improve outcomes.
Requirements:
  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
  • One year prior professional nursing experience as an LPN (CA) or Current LPN Licensure in LA without restrictions and at least one year of experience working as an LPN (LA).
