Job Title: Licensed Practical Nurse - Home Health

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Ochsner Home Health of Raceland. As a LPN, you will provide high-quality patient care and perform technical skilled care in the patient's home.

Responsibilities:
  • Provide quality patient care and perform technical skilled care in the patient's home.
  • Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues or needs.
  • Document patient visits thoroughly and completely per policy and payer requirements.
  • Actively participate in the performance improvement plan and work to reduce patient hospitalizations and improve outcomes.
Requirements:
  • Current LPN licensure in the state of practice.
  • One year of experience working as an LPN, with a minimum of one year of experience in home health care.
  • CPR certification required.
