Job Summary

We are seeking a highly skilled and compassionate Support Specialist to join our team at Ascentria Care Alliance. As a Support Specialist, you will play a vital role in providing in-home supports to adults with intellectual and developmental disabilities (ID/DD) and their families.

Key Responsibilities
  • Establish and maintain positive relationships with individuals served, family members, and other stakeholders.
  • Assist with daily living activities, such as household maintenance, personal care, and hygiene, food shopping and preparation, budgeting, and leisure activities.
  • Provide ongoing support, guidance, and role modeling to individuals to improve their personal responsibility, social skills, community living skills, and behavior.
  • Assist in the development and implementation of Individual Service Plans (ISPs) and Behavioral Support Plans.
  • Coordinate services and provide crisis intervention as needed.
  • Communicate program participant progress, mental status, and any changes in a timely and accurate manner.
  • Attend and participate in meetings and trainings as assigned.
  • Assist individuals to access and provide transportation to appointments, work, and recreational activities.
Requirements
  • High school diploma or equivalent required.
  • Valid driver's license and acceptable driving record required.
  • Ability to work independently and in group environments.
  • Bachelor's degree strongly preferred, but will substitute education for experience.
