Community Integration Manager

Overview: The Community Integration Manager plays a crucial role in delivering comprehensive case management services to individuals facing chronic homelessness within a supportive housing initiative.

Key Responsibilities:

  • Manage a caseload of approximately 20 clients, offering tailored case management support as required.
  • Collaborate effectively within a multidisciplinary team to ensure clients maintain stable housing through the application of trauma-informed practices and harm reduction techniques.
  • Assist clients in finding suitable housing options, securing necessary furnishings, and facilitating their transition into new residences.
  • Utilize transportation resources to support clients in engaging with community activities.
  • Guide clients in reintegrating into the community and achieving independent living.
  • Connect clients with relevant community resources and aid in the development of self-directed service plans.
  • Provide a diverse range of client services, including counseling, referrals, life skills training, employment support, and assistance with financial management and health care.
  • Support clients in obtaining necessary entitlements.
  • Work in partnership with treatment providers and community organizations.
  • Engage in collaborative meetings with service providers to help clients reach their objectives.
  • Maintain organized client records and assist in the completion of program-related documentation.
  • Ensure all program documentation is completed accurately and promptly.
  • Compile and input client data for reporting purposes, ensuring accuracy in the Homeless Management Information System (HMIS).
  • Collaborate with property management and landlords to assist clients in adhering to lease agreements, making timely rent payments, and maintaining their living spaces.
  • Provide crisis intervention and emergency support as needed.
  • Act as a representative of the organization, participating in external meetings and events, and taking on leadership roles such as committee involvement, training, or mentoring colleagues.
  • Perform additional duties as assigned.

Qualifications:

  • Bachelor's or Master's degree in a relevant human services discipline.
  • Practical experience in social and human services, particularly with diverse populations.
  • Understanding of case management principles, housing-first models, trauma-informed care, and harm reduction strategies.
  • Experience working with individuals facing addiction, mental health challenges, and co-occurring disorders.
  • Ability to foster positive relationships with both colleagues and clients.
  • Proficient in advocating for client needs.
