Care Manager Job Summary

Catholic Charities Family Community Services is seeking a skilled Care Manager to join our team. As a Care Manager, you will provide intake and ongoing care management services to individuals living in Monroe, Finger Lakes, or Southern Tier Regional Counties who have been diagnosed with one or more chronic illnesses.

Key Responsibilities

  • Conduct necessary screening and follow-up for clients to proceed with intake in applicable programs.
  • Provide client-guided care management services to assigned caseload, maintaining accurate and verifiable documentation of all services rendered.
  • Complete comprehensive and detailed assessments, reviews, crisis plans, and care plans as required by standards set by DOH and lead Health Home contractors.
  • Demonstrate cultural competency and recognize diversity through supporting and encouraging the strengths of each individual served.
  • Provide a meaningful core service for each client assigned monthly and as needed, to ensure all care plan goals are addressed to the satisfaction of the clients served.
  • Maintain accessibility for clients during the workday and provide linkage/referrals and coordination of resources/support for medical care, mental health, social, recreational, and housing/basic needs, etc.
  • Remain current in the field and support and implement best practice service models within the program.
  • Make independent decisions and recommendations regarding immediate risk factors presented and provide crisis intervention and follow-up as needed.
  • Provide on-call availability via cell phone to Health Home program as needed and serve as client advocate.
  • Facilitate the development of a Care Team to carry out individualized and strength-based Plan of Care through regular case conferences and case reviews.
  • Participate in staff meetings and other Care Coordination department activities and ensure compliance with all applicable local, State, Federal, and Health Home Network regulations and agency policies.
  • Participate in mandatory ongoing training and professional development offered and represent Catholic Charities Family Community Services on appropriate boards and committees as assigned by CC Director.

Qualifications

Candidates must have an Associates degree in Human Services or related field and two years of paid experience working with persons with disabilities and/or chronic illnesses. A combination of education and experience may be substituted at the discretion of the Executive Director. Bilingual (English/Spanish) is required for some positions, and additional compensation will be offered if required.
