Job Description

Core Medical-Perm is seeking a skilled Home Health Physical Therapist to join their team in Greenwich, Connecticut.

Key Responsibilities:
  • Provide physical therapy services to patients in their homes
  • Develop and implement individualized treatment plans to improve patients' functional abilities
  • Collaborate with healthcare teams to ensure comprehensive care

Requirements:
  • Current licensure as a Physical Therapist in the state of Connecticut
  • Experience working in home health or a similar setting
