About the Role:

The House Calls Nurse Practitioner will provide high-quality, personalized medical care to patients in their homes, assisted living facilities, or independent living communities. As part of our interdisciplinary care team, you will work collaboratively with other healthcare professionals to assess patient needs, develop comprehensive care plans, and coordinate services to ensure optimal health outcomes.

Responsibilities:
  • Evidence-Based Practice: Plan, direct, and evaluate patient medical needs, ensuring evidence-based care that addresses chronic and acute conditions.
  • Patient Assessment and Treatment: Conduct thorough examinations, diagnose and treat episodic illnesses, minor injuries, and other health concerns.
  • Collaboration and Communication: Work closely with caregivers, family members, and other healthcare professionals to maintain open lines of communication and ensure seamless care coordination.
  • Medical Record Management: Ensure accurate and up-to-date documentation of patient records, maintaining confidentiality and adhering to HIPAA guidelines.
Requirements:
  • Licensure and Certification: Active state NP licensure, current NP National Board Certification, and active state DEA licensure.
  • Education and Experience: Master's degree in Nursing (MSN), five or more years of nursing experience, and at least two years of nurse practitioner experience preferred.
  • Interpersonal and Technical Skills: Strong verbal and written communication skills, excellent time management, and proficiency in EMR systems, Microsoft Office applications, and computer-based data management programs.
  • Vehicle Requirements: Licensed driver with valid automobile insurance.
