Responsibilities

As a Compassionate Patient Care Specialist, you will be responsible for providing patient-centered care to individuals in their homes or other settings. This will include:

  • Assessing patients' physical, emotional, and social needs
  • Developing and implementing individualized care plans
  • Providing skilled nursing care, including wound care, medication management, and mobility assistance
  • Monitoring patients' progress and adjusting care plans as needed

Requirements

To be considered for this role, you must have:

  • An active GA nursing license or compact
  • Recent (PPD) TB skin test or chest X-ray
  • BLS Provider CPR certification
  • A valid driver's license
  • The ability to pass a background check
