Position Overview:
Riverside Health System is actively looking for a Pharmacist Specialist in Long-Term Care (LTC) to join our labor pool as needed.

Qualifications:

  • Doctor of Pharmacy (Pharm D) or Bachelor of Science in Pharmacy from an accredited institution (Required)
  • A minimum of 2 years of experience in clinical pharmacy (Required)
  • At least 1 year of experience in gerontology or long-term care (Required)
  • Current licensure as a Pharmacist in Virginia (Required)
  • Valid driver's license and a satisfactory driving record required upon hiring and must be maintained throughout employment. Reliable transportation to various facilities is also required (Required)
Key Responsibilities:
As a Pharmacist Specialist, you will oversee the pharmacy operations within long-term care facilities, develop pharmacy policies and procedures specific to these settings, and ensure compliance with state regulations. Your role will include providing essential pharmaceutical and clinical services to enhance the quality of care for residents.

Primary Duties:
  • Review patient medication profiles to ensure appropriate therapy and dosages, providing up-to-date drug information.
  • Assist in the development and management of formularies and protocols pertinent to specialty areas.
  • Oversee all aspects of pharmaceutical care at the assigned location, including supervision of pharmacy technicians.
  • Monitor patient outcomes, resolve data discrepancies, address specialty pharmacy inquiries, and promote services to affiliated practices.
  • Conduct pharmacotherapeutic monitoring and deliver pharmaceutical care tailored to the needs of each patient, considering their age and clinical condition.
  • Provide comprehensive drug information to patients, healthcare providers, and other professionals.
  • Engage regularly with physicians, nurses, and healthcare teams to optimize pharmaceutical care.
  • Participate in pharmaceutical research initiatives within your area of expertise.
