- Document and Communicate: Maintain accurate and detailed records of patient care, communicating effectively with healthcare teams and patients to ensure continuity of care.
- Education: Graduate of an accredited school of nursing (BSN preferred) or Masters Degree in Social Work (MSW) required, or demonstrated success in a CHRISTUS Care Manager I position for at least 5 years.
- Experience: Two or more years of clinical experience, with one year in the acute care setting preferred.
- Licenses and Certifications: RN or LMSW in the state of employment required for new hires, LBSW accepted for associates with 5+ years of demonstrated success and experience in the CHRISTUS Care Manager I role.
