About This Role

As a Care Manager II at Christus Health, you will play a vital role in ensuring seamless patient progression through the continuum of care. This position requires collaboration with patients, families, physicians, and multidisciplinary team members to develop and implement effective care plans.

Key Responsibilities
  • Coordinate Patient Care: Work with patients, families, and healthcare teams to identify and address individual needs, ensuring timely and effective care transitions.
  • Develop and Implement Care Plans: Collaborate with physicians and other healthcare professionals to create comprehensive care plans that meet patient needs and promote optimal outcomes.
  • Manage Patient Flow: Identify and address barriers to efficient patient flow, ensuring smooth transitions and minimizing length of stay.
  • Advocate for Patients: Serve as a patient advocate, negotiating with payors, agencies, and vendors to secure necessary resources and support.
  • Document and Communicate: Maintain accurate and detailed records of patient care, communicating effectively with healthcare teams and patients to ensure continuity of care.
Requirements
  • Education: Graduate of an accredited school of nursing (BSN preferred) or Masters Degree in Social Work (MSW) required, or demonstrated success in a CHRISTUS Care Manager I position for at least 5 years.
  • Experience: Two or more years of clinical experience, with one year in the acute care setting preferred.
  • Licenses and Certifications: RN or LMSW in the state of employment required for new hires, LBSW accepted for associates with 5+ years of demonstrated success and experience in the CHRISTUS Care Manager I role.
