Job Summary
As a Clinical Nurse Liaison at HealthMap Solutions LLC, you will play a vital role in developing and maintaining long-term relationships with healthcare providers, office staff, and patients enrolled in our Kidney Management Program. Your primary responsibility will be to coordinate performance improvement activities and care management to enhance health outcomes.

Responsibilities
  • Establish strong relationships with healthcare providers, their staff, and patients to ensure positive engagement and performance within our program.
  • Develop new business relationships that improve provider and patient engagement and manage assigned caseloads.
  • Analyze provider-specific data to identify opportunities for improving health outcomes.
  • Educate providers on best practices, process improvements, and interventions related to Kidney Health Management and HEDIS/STAR measures.
  • Identify opportunities to educate providers on Chronic Kidney Disease, End-Stage Renal Disease, Renal Replacement Therapies, etc.
  • Partner with physicians and staff to identify patients who would benefit from care navigation support, conduct outreach, and engage patients in the program.
  • Educate patients on kidney health, related comorbid conditions, and renal replacement therapy.
  • Serve as a patient advocate, utilizing community resources and programs, and facilitating communication between the patient, support network, treating physician, and ancillary providers.
  • Maintain thorough documentation of provider and patient interactions to ensure consistency, coordination, and compliance with NCQA standards.
  • Ensure timely delivery of reports to internal and external stakeholders.
