About the Role

We are seeking a highly skilled and compassionate Service Coordinator to join our team at the State of Nebraska. As a Service Coordinator, you will play a critical role in supporting individuals with developmental disabilities and their families.

Key Responsibilities
  • Coordinate and oversee the delivery of services to individuals with developmental disabilities
  • Conduct assessments and eligibility determinations to ensure individuals receive the necessary support
  • Develop and implement personalized plans of services and supports to meet the unique needs of each individual
  • Provide referrals to community resources and services to support individuals and their families
  • Monitor the provision of services to ensure the health, welfare, and safety of those we support
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in a related field, such as education, psychology, social work, or human services
  • Professional experience in services or programs for individuals with intellectual or other developmental disabilities
  • Strong communication and interpersonal skills, with the ability to work effectively with individuals, families, and community partners
  • Experience with developing person-centered plans, completing assessments, and implementing habilitation programs
  • Proficiency in Microsoft Office, including Word, Excel, and Outlook
