Job Summary

The SSVF Case Manager plays a vital role in assisting Veterans who are homeless or at-risk of homelessness to end their housing crisis. This position requires a strong understanding of the challenges faced by Veterans and their families, as well as the ability to provide supportive services and education to enhance their independent living skills.

Key Responsibilities
  • Provide outreach to the community to increase awareness of SSVF and recruit Veteran families to the program
  • Conduct needs assessments and develop/monitor case plans for participants
  • Establish linkages with local agencies and service providers to provide health care services and daily living services to participants
  • Provide referrals and coordination with VAMCs and other service providers to assist participants in obtaining VA benefits and other public benefits
  • Assist participants in obtaining and coordinating supportive services, including personal financial planning, transportation, income support, fiduciary and representative payee services, legal services, child care, and housing counseling
  • Complete required documentation and enter data into the Homeless Management Information System (HMIS)
  • Participate in Performance Quality Improvement (PQI) duties and Critical Time Intervention (CTI) evidence-based model
Requirements

This position requires a Bachelor's degree in social work or a related field, or at least five years of experience in the field with no degree. A demonstrated ability to provide a reasonable combination of family needs assessment, knowledge and ability to access local resources, advocacy, counseling, and working cooperatively with local service providers is essential. Excellent verbal and written communication skills, high degree of organization, and creative problem-solving skills are also required.
