Salary:$65,000 - $85,000 per year

Job Description:

We are seeking an experienced MDS Coordinator (RN License Required) to join our interdisciplinary care planning team. As a key member of our staff, you will be responsible for maintaining accurate and up-to-date resident assessment data, coordinating with various disciplines to ensure comprehensive care plans, and verifying timely submission and approval of MDS records.

Responsibilities:

  • Maintain and follow schedules of due dates for all MDS assessments.
  • Coordinate completion of MDS by all relevant disciplines.
  • Review MDS data for accuracy and collaborate with staff as needed to ensure precision.
  • Complete MDS records and input into computer systems.
  • Verify timely transmission and approval of MDS records by state authorities.
  • Correct rejected records and prepare them for re-submission.
  • Conduct CAAs according to state guidelines.
  • Develop Resident Care Plans (RCPs) in collaboration with the interdisciplinary care planning team.
  • Schedule and conduct regular RCP meetings.
  • Communicate RCPs to relevant staff members and notify supervisors of any non-compliance.
  • Collaborate with internal and external departments to ensure understanding and compliance with RCPs.
  • Perform other related duties as directed by the DON and/or Administrator.

Requirements:

  • Registered Nurse (RN) degree from an accredited School of Nursing and current, valid RN license from the North Carolina Board of Nursing.
  • Ability to read, understand, and adhere to personnel, departmental, and facility policies and procedures, as well as local, state, and federal requirements.
  • Experience with MDS-RAP and Care Planning functions.
  • Prefer experience with RUG-IV and MDS 3.0.
