The Care Connections Nurse Practitioner role at Molina Healthcare focuses on delivering high-quality, patient-centered care in the home, community, and nursing facility settings. As a key member of our team, you will provide essential medical services to patients in their preferred environment, promoting health and well-being through preventive primary care.
Key Responsibilities- Conduct comprehensive medical assessments, order diagnostic tests, and develop treatment plans to address patients' needs.
- Provide medical care to patients of all ages, including adults, women's health, pediatric, and geriatric populations.
- Collaborate with healthcare teams to ensure seamless care coordination and reduce hospital readmission rates.
- Participate in community-based Pop Up Clinics to build relationships with patients and address health care gaps.
- Drive up to 120 miles a day to various locations within the assigned region, with occasional overnight stays for Extended Mileage Special Projects.
- Master's degree in family health from an accredited nursing program.
- Active and unrestricted national certification from a recognized organization (e.g., American Academy of Nurse Practitioners or American Nurses Credentialing Center).
- Current state-issued license to practice as a Family Nurse Practitioner.
- Basic Life Support for Healthcare Professional certification.
- Unrestricted driver's license.
- 3-5 years of experience as a Registered Nurse and/or Nurse Practitioner, preferably in home health, community health, or public health settings.
- Previous experience in home health, especially in managing chronic conditions.
- Fluency in a language in addition to English.
- Flexible scheduling with 4x10-hour or 5x8-hour shifts.
- $30,000 nursing loan repayment over 3 years.
- $2,000 annual CME allowance.
- Home office stipend, mileage reimbursement, and cell phone.
- Tuition/Certification Reimbursement - $5,250 annually.
