Job SummaryThe Care Connections Nurse Practitioner plays a vital role in delivering high-quality, patient-centered care in the home, community, and nursing facility settings. As a key member of our team, you will provide comprehensive medical assessments, develop treatment plans, and coordinate care to ensure the best possible outcomes for our patients.
Key Responsibilities- Provide general medical care and care coordination to adults, women's health, pediatric, and geriatric patients
- Perform comprehensive evaluations, including history and physical exams, to identify gaps in care and prevent complications
- Address chronic and acute primary care complaints, and determine medical urgency
- Establish and document reasonable medical diagnoses, and seek specialty consultation as needed
- Order and perform diagnostic laboratory and radiology testing, using diagnostic tests judiciously and appropriately
- Create and implement a medical plan of care, and provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization
- Perform face-to-face visits in various settings, including home, skilled nursing facilities, and public locations
- Participate in community-based Pop Up Clinics to build relationships with the community and address gaps in healthcare
- Drive up to 120 miles a day to various locations within the assigned region, with occasional drives beyond 120 miles for Extended Mileage Special Projects
- Master's degree in family health from an accredited nursing program
- Advanced computer skills, including proficiency with Word, Excel, and Electronic Medical Records
- Active and unrestricted national certification from a recognized organization, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center
- Current state-issued license to practice as a Family Nurse Practitioner
- Current Basic Life Support for Healthcare Professionals certification
- Current unrestricted driver's license
- 3-5 years of experience as a Registered Nurse and/or Nurse Practitioner, ideally in a home health, community health, or public health setting
- Previous experience in home health as a licensed clinician, especially in management of chronic conditions
- Experience working with underserved populations facing socioeconomic barriers to healthcare
- Fluency in a language in addition to English
- Immunization and point-of-care testing skills
- Flexible scheduling with either 4x10-hour or 5x8-hour shifts
- $30,000 nursing loan repayment over 3 years
- $2,000 annual CME allowance
- Home office stipend, mileage reimbursement, and cell phone
- Tuition/Certification Reimbursement - $5,250 annually
