We are a Christian hospice dedicated to providing high-quality clinical care and the love and salvation of Jesus Christ. As a Registered Nurse Case Manager, you will play a vital role in planning, organizing, and directing hospice care with a focus on community health education and experience.
Key Responsibilities:- Conduct comprehensive assessments of patients and families to determine hospice needs and develop individual care plans that establish goals in collaboration with the patient and family.
- Coordinate patient care for assigned caseloads, ensuring continuity and quality of care.
- Counsel patients and families on meeting nursing and related needs, providing health care instructions as appropriate.
- Assist patients with activities of daily living and facilitate efforts toward self-sufficiency and optional comfort care.
- Document accurate and relevant clinical notes regarding patient condition and care provided.
- Communicate with physicians and hospice care teams regarding patient needs and report changes in patient condition.
- Provide a safe environment for patients and arrange necessary equipment and services.
- Graduate of an accredited school of nursing with 1-2 years of recent acute care experience in an institutional setting and 1-2 years of recent experience in home care.
- Bachelor's degree with one year of hospice care experience preferred.
- Self-directing with ability to work with little direct supervision, yet as a member of a complete interdisciplinary care team.
- Understanding of the philosophy of hospice care.
- Able to lift, position, and/or transfer patients, and lift supplies and equipment.
- Willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs.
- Mileage reimbursement plan.
