Job Summary

We are seeking dedicated and compassionate individuals to join our team as Direct Care Workers. As a Direct Care Worker, you will play a vital role in providing support and care to individuals with developmental and physical disabilities. Your primary responsibility will be to implement behavior programs and other habilitation programs, while also facilitating and promoting community integration and safety.

Key Responsibilities
  • Ensure individuals' medical health issues are addressed, including scheduling doctor's appointments, providing transportation to appointments, and following through on doctor's orders.
  • Assist individuals in daily activities, including daily hygiene functions.
  • Implement behavioral goals, teaching strategies, monitoring procedures, exercise programs, and other team-recommended programs and procedures.
  • Assist in the completion of individual assessments.
  • Model positive adult roles in the community and foster community belonging through person-centered activities.
  • Supervise and assist in the preparation of food for individuals following special prescribed diets and food consistency.
  • Complete hourly safety checks on individuals during overnight shifts.
Requirements
  • High school diploma or general education degree (GED); 3 to 6 months-related experience and/or training, or human service training certificate.
  • Valid Connecticut Driver's License.
  • Ability to obtain Department of Developmental Services - Medication Certification within six months of hire.
  • CPR and First Aid certification.
