About the Job

Ontario Center is seeking a skilled LPN Supervisor to join our team in Canandaigua, NY.

Key Responsibilities:
  • Conduct regular facility rounds to ensure a smooth workflow.
  • Monitor the assignments of aides to guarantee proper work distribution.
  • Supervise medication assistance times to ensure accurate recording and follow-up as needed.
  • Intervene in resident conflicts, outbursts, and behavior issues in a timely manner.
  • Facilitate communication to address medical and mental health situations promptly.
  • Ensure all incident and accident reports are completed and summarized for the EHP Coordinator/Administrator.
  • Attend to late admissions and re-admissions, handling paperwork and medication changes as necessary.
  • Communicate shift activity through 24-hour reports for review by the Enriched Housing Program Coordinator/Administrator.
  • Facilitate communication for environmental situations, contacting the EHP Coordinator/Administrator when necessary and recording details for review.
Requirements:
  • Ability to work effectively as a team member.
  • Valid state LPN license.
  • Good standing with the State Registry.
