About the Position

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct comprehensive health assessments in the comfort of patients' homes, skilled facilities, and through telehealth visits. Your expertise will help identify health risks and provide personalized care plans to improve patient outcomes.

Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
  • Provide health education and counseling to patients and their families
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Document patient information accurately and efficiently
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records

Travel Requirements

As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to travel to various locations to conduct home visits and other clinical activities. A valid driver's license and reliable transportation are required.
