About Meadow Park Rehabilitation and Health Care Center

We are seeking a skilled and compassionate RN Charge Nurse to lead our subacute unit. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and promoting a positive work environment.



Responsibilities:
  • Lead Individualized Resident Care

Develop and implement personalized care plans that address the unique needs and goals of each resident, promoting optimal health and well-being.


Collaborate with Interdisciplinary Team Members

Work closely with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and effective problem-solving.


Supervise and Mentor Nursing Staff

Provide guidance, support, and feedback to assigned team members, promoting professional growth and development.


Monitor Resident Conditions and Administer Medications

Conduct regular assessments and administer medications as prescribed, ensuring the highest level of patient safety and comfort.


Manage Unit Operations and Resources

Oversee the efficient use of resources, including staff, supplies, and equipment, to maintain a smooth and productive work environment.


Participate in Quality Improvement Initiatives

Contribute to ongoing quality improvement efforts, identifying areas for improvement and implementing evidence-based practices to enhance patient outcomes.



Requirements:
  • Licensed as a Registered Nurse in the State of NY
  • Prior Experience in Long-Term Care Nursing
  • Knowledge of Electronic Medical Records (EMR) Systems
  • BCLS, ACLS, or PALS Certification
  • Excellent Communication and Interpersonal Skills
